No, we did not rent a car. Most national agencies are available at the airport or in Cancun. You can rent from National in the lobby of the Royale. My insurer, State Farm, does not cover you in Mexico, you must purchase Mexican auto insurance. I've been told by others that Mexican insurance is mandatory when renting. I have heard and observed that some customs are different, for example, in making a left hand turn (like a U-turn), you activate your left hand turn signal and pull over to the RIGHT to let those behind you pass. Then you proceed across the lanes and turn left. I saw this everytime we were on the highway by our shuttle and bus drivers! Another couple told me last year they parked their car in Cozumel without realizing that that lot has a closing time! There were no signs posted...how were they supposed to know? Anyways, it took many anguished hours and $$$ taking to kind strangers who made calls on their behalf to get the lot opened to retrieve their car. Needless to say, their advice is "don't do it".
